What the words mean
The ladder:
the five questions, in order, under the five numbers. 50%, 25%, 12%, 5% and 1%, the same every day, and the first is the one most people get
The tier:
how hard a question is meant to be. Each one lands under the number nearest its own solve rate, so the 1% always holds the hardest thing we have got
The rate:
the share of players who actually got that question right, shown once you have answered it. It is measured from what people did, not decided by whoever wrote the question, and it moves as more people play
